Students wishing to enroll at Crooked Creek Elementary School for the first time will need to bring an official birth certificate, proof of residency, and immunization records. Forms that will need to be completed upon enrollment, and again at the beginning of every school year, include the following:

• Unit Enrollment Card (blue tagboard, 8.5″ x 6.5″)
• MSD Washington Township School Health Services – Student Health History and on the back: Student Information Emergency Card (This form also has field trip and media permission information on it.)
• Code of Conduct (green tagboard)
• Parent/School Partnership Contract (lilac, full page, front & back)
• Home Language Survey to determine the student’s status as a language minority (during initial enrollment only)

All information gathered on these forms is strictly confidential, and only shared with the pertinent personnel.
